• Parent Proof of Address (current utility bill, mortgage/lease agreement, or official government mail)

• Student Birth Certificate

• Student Immunization Records

• Custody Papers (if relevant)

• Parent Photo ID

Out of District Enrollment:
Out of District enrollment applications are continuously accepted throughout the school year. The cutoff date for 2023-24 out of district enrollment applications is reported to and published by IDOE as May 22, 2024 per IC 20-26-11-32.
